Abstract Introduction Understanding all the benefits of bimodality with self-assessment questionnaires on the effect of hearing on quality of life is still necessary. Objective To present whether bimodality still offers hearing benefits to the population who uses acoustic stimulation associated with electrical stimulation. Methods The present study included 13 participants aged between 16 and 80 years old who were users of cochlear implants from Cochlear Corporation and hearing aids. All patients underwent the Hearing in Noise Test, and their visual analog scale score was obtained. Four-tone means were collected, and the participants answered the Speech, Spatial and Hearing Qualities questionnaire. Results Bimodal users had an average sentence recognition rate of 76.0% in silence and 67.6% in fixed noise, and the signal-to-noise ratio in adaptive noise was +2.89dB. In addition, a lower level of difficulty was observed in the test using the visual analog scale. The domain with the highest average was auditory qualities (6.50), followed by spatial hearing (6.26) and hearing for speech (5.81). Individuals with an average between 50 and 70 dB of hearing level showed better sentence recognition in silence and noise. Conclusion Bimodal stimulation showed benefits for users with different degrees of hearing loss; however, individuals who presented greater hearing residue had better performance in speech recognition with noise and in silence in addition to a good perception of hearing quality.

Objetivos: conocer el perfil y comprender el contexto descrito en las publicaciones sobre la judicialización del acceso a tecnologías en salud en Brasil. Metodología: revisión sistemática de métodos mixtos. Se consultaron 5 bases de datos y se incluyeron artículos que presentaran datos de procesos judiciales demandando tecnologías en salud del SUS en Brasil, o que analizaran este fenómeno, publicados en portugués, inglés y español, hasta 2019. La calidad metodológica fue evaluada con un enfoque convergente segregado. La caracterización de los datos de los estudios y el análisis e integración de la evidencia cuali-cuantitativa se realizó usando la síntesis realista. Resultados: 27 estudios fueron incluidos en la revisión. Se identificaron 76.666 procesos judiciales en que se solicitaba alguna tecnología en salud, en su mayoría medicamentos (65%); la mayoría de los procesos presentaron una representación jurídica privada (68%) y la mayoría de las acciones fue contra los estados (65%). Los enfoques de análisis de judicialización más frecuentes fueron Social Positivo (32%) y Normativo Negativo (32%), seguidos de 29% para el abordaje de Normativo Positivo. El abordaje Social Negativo (7%) fue el menos frecuente. Conclusiones: se identificó que existen evidencias cuantitativas que soportan las evidencias cualitativas. Estas demuestran que el desconocimiento del Poder Judicial sobre las políticas de salud lleva a un aumento de las desigualdades en salud; al fomento del paternalismo del Estado, así como que el aumento de acciones judiciales explica la necesidad de la búsqueda de alternativas para acceder a tecnologías en salud.


Objectives: to know the profile and understand the context described in publications on the judicialization of access to health technologies in Brazil. Methods: systematic review of mixed methods. Five databases were consulted, and articles were included that presented data from lawsuits demanding health technologies from the SUS in Brazil, or that analyzed this phenomenon, published in Portuguese, English and Spanish, until 2019. The methodological quality was evaluated using the convergent segregated approach. The characterization of data from the studies and the analysis and integration of qualitative-quantitative evidence were conducted through realistic synthesis. Results: 27 studies were included in the review. A total of 76,666 lawsuits were identified in which some health technology was requested, mainly medicines (65%); most lawsuits had private legal representation (68%) and most lawsuits were against the states (65%). The most frequent judicialization analysis approaches were Positive Social (32%) and Negative Normative (32%), followed by 29% for the Positive Normative approach. The Social Negative approach (7%) was the least frequent. Conclusions: it was identified that there is quantitative evidence that supports qualitative evidence. These demonstrate that the Judiciary's lack of knowledge about health policies leads to an increase in health inequalities; to the promotion of state paternalism, as well as the fact that the increase in lawsuits explains the need to seek alternatives for accessing health technologies.

Foram consultadas cinco bases de dados e incluídos artigos que apresentassem dados de processos judiciais demandando tecnologias em saúde do SUS no Brasil, ou que analisassem esse fenômeno, publicados em português, inglês e espanhol, até 2019. A qualidade metodológica foi avaliada com a abordagem convergente segregada. A caracterização dos dados dos estudos e a análise e integração das evidências qualitativos-quantitativas foram realizadas por meio da síntese realista. Resultados: 27 estudos foram incluídos na revisão. Foram identificados 76.666 processos judiciais em que foi solicitada alguma tecnologia em saúde, principalmente medicamentos (65%); a maioria dos processos apresentou representação legal privada (68%) e a maioria das ações foi contra os estados (65%). As abordagens de análise de judicialização mais frequentes foram Social Positiva (32%) e Normativa Negativa (32%), seguidas de 29% para a abordagem Normativa Positiva. A abordagem Social Negativa (7%) foi a menos frequente. Conclusões: identificou-se que há evidências quantitativas que sustentam as evidências qualitativas. Estes demonstram que o desconhecimento do Judiciário sobre as políticas de saúde leva ao aumento das desigualdades em saúde; à promoção do paternalismo estatal, bem como o fato de que o aumento das ações judiciais explica a necessidade de buscar alternativas de acesso às tecnologias em saúde.

Introdução: A insuficiência renal aguda (IRA) ocorre em cerca de 5% das hospitalizações e até 30% das internações em UTI, possuindo taxas de mortalidade entre 15% e 60%. Objetivo: O objetivo do presente trabalho foi analisar diferentes protocolos de indução da IRA em modelo animal, buscando a padronização do modelo para o estudo da relação bidirecional com a periodontite apical (PA). Métodos: Foram utilizados vinte e quatro ratos divididos em três grupos (n=8): G1) administração de gentamicina na dosagem de 80mg/Kg/dia durante cinco dias; G2) administração de gentamicina na dosagem de 100mg/kg/dia durante oito dias; e G3) administração de gentamicina na dosagem de 100mg/kg/dia durante dez dias. A PA foi induzida em todos os grupos por meio da exposição pulpar dos primeiros e segundos molares superiores e inferiores do lado direito. Após trinta dias da exposição pulpar, os animais foram eutanasiados; os rins, coletados para análise histológica em HE; e as maxilas e mandíbulas, removidas para análise radiográfica. Resultados: A presença da PA foi confirmada em todos os espécimes pelo exame radiográfico. O grupo G3 apresentou índice de mortalidade de 75%, enquanto G1 e G2 não apresentaram mortalidade. Histologicamente, o tecido renal do grupo G2 apresentou alterações teciduais, como dilatação e necrose tubular mais severa, quando comparado ao grupo G1 (p<0,05). Conclusão: Diante do alto índice de mortalidade observado em G3 e dos achados histológicos observados em G1 e G2, o protocolo indicado para o estudo da relação bidirecional com a periodontite apical é o de 100mg/kg/dia durante oito dias (AU).


Introduction: Acute Renal Failure (ARF) occurs in about 5% of hospitalizations and up to 30% of ICU admissions, with mortality rates between 15% and 60%. The objective of this study was to analyze different protocols for the induction of IRA in an animal model, seeking the standardization of the model for the study of bidirectional relationship with apical periodontitis (AP). Methods: Twenty-four rats were divided into three groups (n = 8): G1 - Administration of Gentamicin at a dose of 80mg/ kg/day for 5 days; G2 - Administration of Gentamicin at the dose of 100mg/kg/day for 8 days; G3 - Administration of Gentamicin at the dosage of 100mg/kg/day for 10 days. The AP was induced in all groups by means of pulp exposure of the first and second upper and lower molars from the right side. After 30 days, the animals were euthanized, the kidneys collected for histological analysis, and the jaws removed for radiographic analysis. Results: The presence of AP was confirmed in all specimens by radiographic examination. The G3 group had a mortality rate of 75% while G1 and G2 had no mortality. Histologically, the renal tissue of the G2 protocol presented tissue changes such as dilation and more severe tubular necrosis when compared to the G1 protocol (p <0.05). Conclusion: In view of the high mortality rate observed in G3 and the histological findings observed in G1 and G2, the indicated protocol is 100mg/kg/day for 8 days to study the bidirectional relationship with apical periodontitis (AU).

ABSTRACT Objective: Hypothalamic inflammation and glial fibrillary acidic protein (GFAP) overexpression in astrocytes are well described in obese animals, as are some cognitive and memory deficits. As the hippocampus plays important roles in the consolidation of information, this investigation aimed to observe the memory function and the astrocyte expression of GFAP in the hippocampus of rats that received either a hypercaloric or a normocaloric diet. Methods: Adult male Wistar rats received a high-fat (cafeteria) or a standard diet for 60 days. On the 61st day, the rats were submitted to the novel object recognition (NOR) test at three and 24 hours after the first contact with objects, to assess short-term and long-term memory, respectively. Thereafter, the rats were euthanized and their brains were collected for GFAP immunohistochemical investigation in the hippocampus (CA1, CA2, CA3 areas) and hypothalamus (periventricular and arcuate nuclei). Astrocytic reactivity was assessed by morphometry. Different white adipose tissue depots and brown adipose tissue were weighed to calculate the adiposity index. Results: The hypercaloric diet increased body weight gain, adiposity index, white adipose tissue weight (epididymal, subcutaneous and retroperitoneal) and brown adipose tissue weight. Rats fed with the hypercaloric diet showed short-term and long-term memory impairments in the NOR test, as well as increased GFAP expression in astrocytes from all analyzed hypothalamic and hippocampal areas. Conclusion: This astrogliosis suggests that the neuroinflammatory response also occurs in the hippocampus and may be involved in the memory losses observed in obese/overweight animals.


RESUMO Objetivo: A inflamação hipotalâmica e a superexpressão da proteína glial fibrilar ácida (GFAP) em astrócitos são bem descritas em animais obesos, assim como déficits cognitivos e de memória. Como o hipocampo desempenha importante papel na consolidação de informações, esta investigação teve como objetivo observar a função da memória e a expressão astrocitária da GFAP no hipocampo de ratos que receberam dieta hipercalórica ou normocalórica. Métodos: Ratos Wistar machos adultos receberam dieta rica em gordura (cafeteria) ou dieta padrão por 60 dias. No 61º dia, os ratos foram submetidos ao teste de reconhecimento de objetos (NOR) 3 e 24 horas após o primeiro contato com os objetos, para avaliação da memória de curto e de longo prazo, respectivamente. Após, os ratos foram eutanasiados e os encéfalos coletados para pesquisa imuno-histoquímica da expressão astrocitária de GFAP no hipocampo (áreas CA1, CA2 e CA3) e no hipotálamo (núcleos periventricular e arqueado). A reatividade astrocitária foi avaliada por morfometria. Diferentes depósitos de tecido adiposo branco e marrom foram pesados para calcular o índice de adiposidade. Resultados: A dieta hipercalórica aumentou o ganho de peso corporal, o índice de adiposidade, o peso do tecido adiposo branco (epididimal, subcutâneo e retroperitoneal) e marrom. Ratos alimentados com dieta hipercalórica apresentaram prejuízos na memória de curto e longo prazo no teste NOR e aumento da expressão de GFAP em astrócitos de todas as áreas hipotalâmicas e hipocampais analisadas. Conclusão: Esta astrogliose sugere que a resposta neuroinflamatória também ocorre no hipocampo, podendo estar envolvida nas perdas de memória observadas em animais obesos/com sobrepeso.

Abstract Introduction The bone-anchored hearing aid (BAHA) is a bone conduction system that transmits the sound directly to the inner ear by surpassing the skin impedance and the subcutaneous tissue. It is indicated for patients with mixed, conductive and unilateral sensorineural hearing loss who did not benefit from conventional hearing aids (HAs). Although the benefits from BAHA are well demonstrated internationally, this field still lacks studies in Brazil. Objective To assess the auditory rehabilitation process in BAHA users through audiological, speech perception and tinnitus aspects. Methods Individuals with hearing loss were assessed before and after the implantation. The participants were subjected to pure tone audiometry in free field, functional gain audiometry, speech perception tests, tinnitus handicap inventory (THI) in open format, and to the visual analog scale (VAS). Results It was found that the participants benefited from the use of BAHA. The difference in the performance of the participants before and after the BAHA surgery was significant in terms of hearing acuity. There was no statistically significant difference in the speech perception tests. The tinnitus assessment showed that 80% of the participants scored slight tinnitus severity in THI after using a BAHA. Eighty percent of the participants classified their tinnitus as absent tomild in the VAS after the surgery. Conclusion Based on the results of the current study, we can conclude that the participants improved both the auditory perception and the tinnitus handicap. (AU)

ABSTRACT Introduction: Endothelial dysfunction may contribute to hypercoagulable and inflammation states presents in renal transplant, chronic kidney disease (CKD) and its causes. These disorders can be evaluated by markers, such as thrombomodulin (TM), von Willebrand factor (vWF) and interleukin 6 (IL-6). Objectives: The aim of this study was to assess TM, vWF and IL-6 in renal transplant recipients (RTR) and associate their plasma levels with primary cause of end-stage renal disease (ESRD) and allograft function. Methods: 160 RTR were grouped according to the primary cause of CKD (G1: glomerulopathy; G2: hypertensive nephrosclerosis; G3: diabetic nephropathy; and G4: other causes/unknown etiology); creatinine plasma levels (C1 < 1.4 and C2 ≥ 1.4 mg/dl); and the estimated glomerular filtration rate (eGFR) (R1< 60 and R2 ≥ 60 ml/min/1.73 m2). TM and vWF were determined by the enzyme-linked immunosorbent assay (ELISA) and IL-6 by flow cytometry. The results were presented as median, minimum and maximum; p-value < 0.05 was considered statistically significant. Results: TM levels were significantly higher in the G1 group compared to the others (G1: 8.38; G2: 5.51; G3: 5.88; G4: 6.33 ng/ml, p < 0.0001), and in the R1 group compared to R2 (R1: 6.65; R2: 6.19 ng/ml, p = 0.02). The concentration of IL-6, measured by the mean fluorescence intensity, was higher in C2 group when compared to C1 (C1: 7.9; C2: 13.35, p = 0.03). There was no difference in vWF levels among groups. TM correlated positively with IL-6 and creatinine, and negatively with eGFR. IL-6 also correlated positively with vWF. Conclusion: TM and IL-6 can be identified as potential markers for evaluating renal graft function. TM was more related to the primary cause of CKD compared to vWF and IL-6.


RESUMO Introdução: A disfunção endotelial pode contribuir para estados de hipercoagulabilidade e inflamação presentes no transplante renal e na doença renal crônica (DRC) e suas causas, podendo ser avaliada por marcadores como trombomodulina (TM), fator de von Willebrand (FvW) e interleucina 6 (IL-6). Objetivos: Avaliar TM, FvW e IL-6 em receptores do transplante renal (RTR) e associar seus níveis com a causa primária de DRC pré-transplante e função do enxerto. Métodos: Foram alocados 160 RTR em grupos de acordo com a causa primária da DRC (G1: glomerulopatias; G2: nefroesclerose hipertensiva; G3: nefropatia diabética; e G4: outras causas/etiologia desconhecida), os níveis plasmáticos de creatinina (C1 < 1.4 e C2 ≥ 1.4 mg/dl) e o ritmo de filtração glomerular estimado (eRFG) (R1< 60 e R2 ≥ 60 ml/min/1.73 m2). A TM e o FvW foram determinados pelo ensaio de imunoabsorção enzimática (ELISA) e a IL-6, por citometria de fluxo. Os resultados foram apresentados como mediana, mínimo e máximo; p < 0,05 foi considerado significativo. Resultados: Níveis de TM foram significativamente maiores no grupo G1 em comparação com os demais (G1: 8,38; G2: 5,51; G3: 5,88; G4: 6,33 ng/ml, p < 0,0001), e no grupo R1 comparado com o R2 (R1: 6,65; R2: 6,19 ng/ml, p = 0,02). A concentração de IL-6, avaliada pela intensidade média de fluorescência, foi maior no grupo C2 quando comparada com o C1 (C1: 7,9; C2: 13,35, p = 0,03). Não houve diferença entre os grupos para o FvW. TM correlacionou-se positivamente com IL-6 e creatinina e negativamente com eRFG. A IL-6 foi positivamente correlacionada com o FvW. Conclusão: TM e IL-6 podem ser apontadas como potenciais marcadores para avaliar a função do enxerto renal. A TM relacionou-se mais com a causa primária da DRC, se comparada com FvW e IL-6.

Abstract The aim of this study was to evaluate the influence of the prophylactic and therapeutic supplementation with omega 3 polyunsaturated fatty acids (w-3 PUFAs) on the lipid profile and periapical bone resorption in rats with apical periodontitis. Forty male rats were divided into groups: control rats (C), rats treated with w-3 PUFAs (C+O), rats with pulp exposure-induced apical periodontitis (AP), and rats with AP treated with w-3 PUFAs (AP+O). The administration of w-3 PUFAs was carried out orally once a day for 15 days before pulp exposure and, subsequently, for an additional 30 days after pulp exposure. AP was induced by exposing pulpal tissues to the oral environment. The samples were collected after 30 days. Triglycerides and cholesterol levels were enzymatically measured using the Trinder method. The jaws were collected and submitted for histological analysis. Two-way analysis of variance and Kruskal-Wallis tests were used for statistical analysis, and the significance was set at p<0.05. The triglyceride levels of the AP group were significantly higher than those of the C, C+O and AP+O groups (p<0.05). However, the difference in the cholesterol levels among the groups was not significant (p>0.05). Rats with AP showed larger areas of bone resorption as well as higher inflammatory intensity compared with rats with AP supplemented with w-3 PUFAs. It may be concluded that the presence of multiple AP foci increased the triglyceride levels. In addition, omega 3 supplementation might reduce these levels in rats with AP, as well as the bone resorption areas of periapical tissues.


Resumo O objetivo deste estudo foi avaliar a influência da suplementação profilática e terapêutica com os ácidos graxos ômega-3 no perfil lipídico e na reabsorção óssea, em ratos com periodontite apical. Quarenta ratos machos foram divididos em grupos: ratos controle (C), ratos tratados com ácidos graxos ômega-3 (C+O), ratos com periodontite apical induzida por meio de exposição pulpar (PA), ratos com PA tratados com ácidos graxos ômega-3 (PA+O). A administração do ômega-3 foi realizada oralmente, uma vez ao dia durante 15 antes da exposição pulpar e, subsequentemente, por mais 30 dias depois da exposição pulpar. A PA foi induzida por meio da exposição do tecido pulpar ao ambiente oral. Após 30 dias, os ratos foram mortos e os níveis de triglicérides e colesterol foram mensurados pelo método enzimático de Trinder. As mandíbulas foram coletadas e submetidas à análise histológica. Análise de variância de dois fatores e teste de Kruskal-Wallis foram utilizados para análise estatística, e o nível de significância foi de p < 0,05. Os níveis de triglicérides do grupo PA foram significativamente maiores que dos grupos C, C+O e PA+O (p<0,05). Entretanto, não houve diferença significativa nos níveis de colesterol entre os grupos (p>0,05). Ratos com PA apresentaram maior área de reabsorção óssea bem como maior intensidade no infiltrado inflamatório comparados aos ratos com PA suplementados com ômega-3. Pode-se concluir que a presença de múltiplos focos de PA aumentou os níveis de triglicérides. Além disso, a suplementação com ômega-3 pode reduzir estes níveis em ratos com PA, bem como a área de reabsorção óssea dos tecidos periapicais.

ABSTRACT Recent studies have demonstrated that curcumin (Cur) has antioxidant, anti-inflammatory and anti-fibrotic effects. Ethidium bromide (EB) injections into the central nervous system (CNS) are known to induce local oligodendroglial and astrocytic loss, resulting in primary demyelination and neuroinflammation. Peripheral astrogliosis is seen around the injury site with increased immunoreactivity to glial fibrillary acidic protein (GFAP). This investigation aimed to evaluate the effect of Cur administration on astrocytic response following gliotoxic injury. Wistar rats were injected with EB into the cisterna pontis and treated, or not, with Cur (100 mg/kg/day, intraperitoneal route) during the experimental period. Brainstem sections were collected at 15, 21 and 31 days after EB injection and processed for GFAP immunohistochemical staining. Astrocytic reactivity was measured in a computerized system for image analysis. In Cur-treated rats, the GFAP-stained area around the lesion was significantly smaller in all periods after EB injection compared to untreated animals, showing that Cur reduces glial scar development following injury.


RESUMO Estudos recentes têm demonstrado que a curcumina (Cur) possui efeitos antioxidantes, anti-inflamatórios e antifibróticos. Sabe-se que a injeção de brometo de etídio (EB) no sistema nervoso central induz a perda oligodendroglial e astrocitária, resultando em desmielinização primária e neuroinflamação. Astrogliose periférica é observada ao redor da lesão com aumento da imunorreatividade à proteína glial fibrilar ácida (GFAP). A presente investigação objetivou avaliar o efeito da Cur sobre a resposta astrocitária após injúria gliotóxica. Ratos Wistar foram injetados com EB na cisterna basal e tratados ou não com Cur (100 mg/kg/dia, via intraperitoneal) durante o período experimental. Amostras do tronco encefálico foram coletadas aos 15, 21 e 31 dias pós-injeção de EB e processadas para estudo imuno-histoquímico para a GFAP. A reatividade astrocitária foi medida em um sistema computadorizado para análise de imagem. Nos ratos tratados com Cur, a área marcada para GFAP foi significantemente menor em todos os períodos pós-injeção de EB, indicando que a Cur reduz o desenvolvimento da cicatriz glial após injúria.

Abstract Introduction Altered lingual frenum modifies the normal tongue mobility, which may influence the stomatognathic functions, resulting in anatomical, physiological and social damage to the subject. It is necessary that health professionals are aware of the process of evaluation, diagnostics and treatment used today, guiding their intervention. Objective To perform a systematic review of what are the treatment methods used in cases of lingual frenum alteration. Data Synthesis The literature searches were conducted in MEDLINE, LILACS, SciELO, Cochrane and IBECS, delimited by language (Portuguese, English, Spanish), date of publication (January 2000 to January 2014) and studies performed in humans. The selection order used to verify the eligibility of the studies were related to: full text availability; review the abstract; text analysis; final selection. Of the total 443 publications, 26 remained for analysis. The surgical approach was used in all studies, regardless of the study population (infants, children and adults), with a range of tools and techniques employed; speech therapy was recommended in the post surgical in 4 studies. Only 4 studies, all with infants, showed scientific evidence. Conclusion Surgical intervention is effective for the remission of the limitations caused by the alteration on lingual frenum, but there is a deficit of studies with higher methodological quality. The benefits of speech therapy in the post surgical period are described from improvement in the language of mobility aspects and speech articulation.

ABSTRACT Propentofylline is a xanthine derivative that depresses activation of glial cells, whose responses contribute to neural tissue damage during inflammation. Ethidium bromide injection into the central nervous system induces local oligodendroglial and astrocytic loss, resulting in primary demyelination, neuroinflammation and blood-brain barrier disruption. Surviving astrocytes present a vigorous reaction around the injury site with increased immunoreactivity to glial fibrillary acidic protein (GFAP). Objective This study aimed to evaluate the effect of propentofylline administration on astrocytic response following gliotoxic injury. Method Wistar rats were injected with ethidium bromide into the cisterna pontis and treated or not with propentofylline (12.5mg/kg/day, intraperitoneal) during the experimental period. Brainstem sections were collected from 15 to 31 days after gliotoxic injection and processed for GFAP immunohistochemistry. Results and Conclusion Results demonstrate that propentofylline decreased astrocytic activation until the 21st day, suggesting that this drug may have a role in reducing glial scar development following injury.


RESUMO A propentofilina é uma xantina que deprime a ativação das células gliais, cujas respostas contribuem para o dano neural durante inflamação. A injeção de brometo de etídio no sistema nervoso central induz a perda oligodendroglial e astrocitária, resultando em desmielinização, neuroinflamação e ruptura da barreira hematoencefálica. Os astrócitos sobreviventes apresentam vigorosa reação ao redor da lesão com aumento da imunorreatividade à proteína glial fibrilar ácida (GFAP). Objetivo Este estudo objetivou avaliar o efeito da propentofilina sobre a resposta astrocitária após injúria gliotóxica. Método Ratos Wistar foram injetados com brometo de etídio na cisterna basal e tratados ou não com propentofilina (12.5mg/kg/dia, intraperitoneal). Amostras do tronco encefálico foram coletadas dos 15 aos 31 dias pós-injeção do gliotóxico e processadas para estudo ultraestrutural e imuno-histoquímico para GFAP. Resultados e Conclusão Os resultados demonstram que a propentofilina reduziu a ativação astrocitária até o 21o dia, sugerindo que essa droga pode atuar na redução da cicatriz glial após injúria.

ABSTRACT INTRODUCTION: The cochlear implant device has the capacity to measure the electrically evoked compound action potential of the auditory nerve. The neural response telemetry is used in order to measure the electrically evoked compound action potential of the auditory nerve. OBJECTIVE: To analyze the electrically evoked compound action potential, through the neural response telemetry, in children with bilateral cochlear implants. METHODS: This is an analytical, prospective, longitudinal, historical cohort study. Six children, aged 1-4 years, with bilateral cochlear implant were assessed at five different intervals during their first year of cochlear implant use. RESULTS: There were significant differences in follow-up time (p = 0.0082) and electrode position (p = 0.0019) in the T-NRT measure. There was a significant difference in the interaction between time of follow-up and electrode position (p = 0.0143) when measuring the N1-P1 wave amplitude between the three electrodes at each time of follow-up. CONCLUSION: The electrically evoked compound action potential measurement using neural response telemetry in children with bilateral cochlear implants during the first year of follow-up was effective in demonstrating the synchronized bilateral development of the peripheral auditory pathways in the studied population.


RESUMO INTRODUÇÃO: O implante coclear tem a capacidade de medir o potencial de ação composto eletricamente evocado do nervo auditivo (ECAP). Para esta verificação utiliza-se uma medida chamada telemetria de respostas neurais. OBJETIVO: Analisar o potencial de ação composto evocado eletricamente, por meio da neurotelemetria de respostas neurais, em crianças usuárias de implante coclear bilateral. MÉTODO: Trata-se de um estudo analítico, prospectivo, de coorte histórica longitudinal. Foram recrutadas seis crianças, com idades entre de 1-4 anos, usuárias de implante coclear bilateral. Estas crianças foram avaliadas em cinco momentos durante o primeiro ano de uso do implante coclear. RESULTADOS: Houve diferença significativa no tempo de acompanhamento (p = 0,0082) e posição do eletrodo (p = 0,0019) na medida de T-NRT. Houve diferença significativa na interação entre tempo de acompanhamento e posição do eletrodo (p = 0,0143) na medida da amplitude das ondas N1-P1 entre os três eletrodos a cada tempo de acompanhamento. CONCLUSÃO: A mensuração do ECAP por meio da NRT nas crianças com implante coclear bilateral durante o primeiro ano de acompanhamento foi uma medida importante para apresentar o desenvolvimento bilateral da via auditiva periférica de forma sincronizada nesta população estudada.

INTRODUCTION: Auditory rehabilitation in children with bilateral severe-to-profound sensorineural hearing loss with cochlear implant has been developed in recent decades; however, the rehabilitation of children with cerebral palsy still remains a challenge to otolaryngology and speech therapy professionals. OBJECTIVE: To verify the effectiveness of cochlear implants in the development of auditory and language skills in children with cerebral palsy. METHODS: A prospective analytical study. The evaluation of auditory responses to speech test was applied to the children in this study at regular intervals following implantation. Standardized tests that assess and quantify the development of auditory and language skills were administered and speech therapy video records and speech therapy files were analyzed. All children went through individually tailored intensive audiological rehabilitation programs following cochlear implantation. RESULTS: Two participants had gradual auditory and language development when compared to other participants who reached advanced levels in hearing and oral language classifications. CONCLUSION: The use of the Cochlear implant enabled participants to reach advanced stages of hearing and language skills in three of the five participants with cerebral palsy in this study. This electronic device is a viable therapeutic option for children with cerebral palsy to help them achieve complex levels of auditory and language skills. .


INTRODUÇÃO: A reabilitação auditiva em crianças com deficiência auditiva neurossensorial severa a profunda bilateral com o Implante Coclear foi consagrado nas últimas décadas, contudo, ainda permanece um desafio para a otorrinolaringologia e a fonoaudiologia a reabilitação do portador de paralisia cerebral. OBJETIVO: Verificar a efetividade do Implante Coclear no desenvolvimento das habilidades auditivas e de linguagem em crianças com paralisia cerebral. MÉTODO: Estudo analítico prospectivo. Foram aplicados testes padronizados que avaliam e quantificam o desenvolvimento das habilidades auditivas e de linguagem. Foram analisadas as filmagens das terapias fonoaudiológicas e os registros descritos ao término de cada sessão de terapia. RESULTADOS: As crianças analisadas apresentaram desenvolvimento auditivo e de linguagem satisfatório quando comparado às demais crianças que alcançaram níveis mais complexos nas categorias de audição e evolução significativa no desenvolvimento da linguagem oral. CONCLUSÃO: O uso do Implante Coclear favoreceu o alcance de etapas avançadas das habilidades de audição e linguagem em três das cinco crianças com paralisia cerebral desse estudo. Esse dispositivo eletrônico tem sido uma opção terapêutica viável para que crianças com paralisia cerebral alcancem etapas complexas no que se refere às habilidades auditivas e de linguagem. .

Objetivo : analisar os fatores que interferem no desempenho de usuários de implante coclear em testes de percepção de fala.Métodos : foram aplicados questionários e testes de percepção de fala em 25 indivíduos, de ambos os sexos, portadores de perda auditiva neurossensorial e usuários de IC unilateral.Resultados : - 48% dos participantes atingiram bom desempenho nos resultados dos testes de percepção de fala, apresentando porcentagens maiores ou iguais a 50 % de acertos. - Os participantes que obtiveram melhores resultados não são necessariamente os que possuem maior idade auditiva. - Dos 12 participantes que atingiram desempenho acima de 50%, 10 (83,3%) foram implantados mais precocemente (até 5 anos). - O uso do AASI na orelha contralateral influenciou significantemente nos índices de reconhecimento das palavras ou sentenças. - Os participantes que iniciaram o uso de AASI mais cedo não foram necessariamente os que apresentaram melhores resultados nos testes.Conclusão : a ativação mais precoce dos IC mostrou-se uma variável importante no desempenho dos testes. - O uso do AASI na orelha contralateral influenciou significantemente nos índices de reconhecimento das palavras ou sentenças.


Purpose : to analyze the relation between speech perception tests in cochlear implant users.Methods : questionnaires and speech perception tests were applied in 25 individuals from both genders with sensorioneural hearing loss and users of unilateral cochlear implant.Results : 48% of the participants had a good performance (50%) on speech perception tests. The participants who had the best results are not the ones with higher auditory age. 12 participants had a score higher than 50% and 10 (83%) of those, did the cochlear implant surgery before 5 years old. The use of hearing aid contralateral to the cochlear implant had a significant influence on the participants performances on tests. The participants who started using hearing aids earlier did not have the best results on the tests.Conclusion : the early activation of the cochlear implant is an important variable on the tests performance. The use of hearing aid contralateral to the cochlear implant had a significant influence on the speech recognition tests.

Introduction: Hearing screening allows the identification of individuals with hearing loss. Aim: To determine the sensitivity and specificity of a portable hearing screening device in middle-aged and older adults using the manufacturer scoring and a scoring system proposed by the researchers. Methods: In this transversal study, participants underwent anamnesis, otoscopy, and hearing screening using portable equipment. After this, a pure tone audiometry was performed, with participants classified into two groups: with and without hearing loss. The sensitivity and specificity of the hearing screening were calculated for the right and left ears using two methods of interpretation: the original method recommended by the manufacturer (criteria 1) and the method proposed by researchers (criteria 2). Results: The sample consisted of 55 individuals, 83.6% (n = 46) of whom were women. Per criteria 1, the sensitivities were 26.3 (right ear) and 21.4% (left ear). The specificity was 100% for both ears. Using criteria 2, the sensitivity was 94.7 (right ear) and 100% (left ear). The specificity was 74.3 (right ear) and 65.9% (left ear). Conclusion: This study showed that the criteria proposed by the manufacturer presented low sensitivity in the hearing screening. O artigo visa apresentar contribuições de Edith Stein para a fundamentação filosófica da Psicologia enquanto ciência rigorosa da subjetividade. São destacados momentos relevantes para situar o berço da Psicologia científica, assim como o surgimento da Fenomenologia. Ambas surgiram como resultado de questões filosóficas, sendo uma delas referente ao tema do psicologismo e à decorrente tendência à naturalização da Psicologia, de modo que a Fenomenologia surge em oposição a tal movimento. A figura de Edith Stein se destaca devido a suas análises do objeto e do enquadramento epistemológico da Psicologia. Partindo da análise das estruturas essenciais da pessoa, ela oferece uma nova concepção de subjetividade, assim como analisa as metodologias científicas que melhor poderiam apreender esse objeto.


The article intends to present Edith Stein's contributions to the philosophical foundation of Psychology as a rigorous science of the subjectivity. Some relevant moments that situate the cradle of scientific Psychology as well as the emerging of Phenomenology are detached. Both disciplines arose from philosophical matters, and one of them refers to the concept of psychologism and the consequent naturalization of psychology. The phenomenological conception emerged in declared opposition to those movements. The figure of phenomenologist Edith Stein stands out due to her analyses of the object and the epistemological framing of Psychology. Based on the analysis of the essential structures of the human person, Stein offers a new conception of subjectivity, as well as analyses the scientific methodologies that could better apprehend such object.

ABSTRACT

The CHARGE association (coloboma of the eyes; heart disease; atresia of the choanae; retarded growth and development; genital hypoplasia/genitourinary anomalies; ear anomalies and/or hearing loss) was first described in 1979 by Hall, and among its main features is hearing loss. This study presents a case aiming to establish relationships between performance on Infant Toddler Meaningful Auditory Integration Scale (IT-MAIS) and Meaningful Use of Speech Scales (MUSS) tests and the analysis of hearing and language categories of a patient diagnosed with CHARGE syndrome, before and after cochlear implant (CI) surgery. Case Report: A 7-year-old girl was diagnosed with CHARGE. She had severe sensorineural hearing loss and was a prelingual unilateral CI user. We analyzed data from the patient's medical records regarding therapies and video recordings. Results: The patient showed positive results in all evaluations after CI. IT-MAIS rose from 5 to 90% following the use of CI. MUSS also rose, from 75 to 72.5%, after use of CI. Classification of Auditory Skills changed from category 1 before use of CI to category 6 after use of CI. Classification of Language Skills changed from category 1 before use of CI to category 3 after use of CI. The CI is an aid but there are many factors in the therapeutic process, and great heterogeneity in individuals diagnosed with CHARGE should be investigated. Conclusion: The development of listening and language skills after CI use was demonstrated by IT-MAIS and MUSS tests, and categorization of speech and hearing in this child with a diagnosis of CHARGE syndrome shows that CI can be an effective technological resource to provide information on hearing as one source for language construction...

ABSTRACT

O alinhamento dental é um importante fator de estética na sociedade moderna. O espaço existente entre os dentes anteriores superiores associado ao freio labial patológico durante o desenvolvimento da dentição tem sido motivo de preocupação entre os pais dos pacientes pediátricos. Quando o freio labial superior se encontra hipertrofiado ou em má posição, pode ser considerado um desvio da normalidade, necessitando de procedimentos cirúrgicos para a sua correção. A proposta deste trabalho foi relatar dois casos clínicos de anomalia de freio labial superior associado a diastema mediano, destacando a ocorrência do fechamento parcial do espaço interincisal após frenectomia. Os pacientes foram atendidos no ambulatório da disciplina Odontopediatria 11 da Universidade Estadual de Feira de Santana e as cirurgias foram executadas com sucesso. Após um ano de acompanhamento, verificaram-se resultados que sugeriram adequada indicação da técnica para o fechamento parcial de diastema interincisal dos casos tratados.


The dental alignment has been an important factor of aesthetics in the modern society. The anterior superior teeth space associated to the pathological labial frenum during the development of the dentition has been a major concern for the pediatric patients' parents. When the superior labial frenum is found hipertrophic or in bad position, a shunting line of normality, needing surgical procedures for its correction can be considered. The purpose of this study was to report two clinical cases of abnormal superior labium frenum associated with midline diastema showing spontaneous partial closing of diastema after frenectomy. The patients were carried through the Odontopediatria II discipline clinic of the Feira of Santana State University and the surgeries were successfully executed. After a year of attendance, the results suggested appropriate indication of the technique for the partial closing of midline diastema of the treated cases.

Significant decrease in human immunodeficiency virus type 1 (HIV-1) vertical transmission has been observed worldwide in centers where interventions such as antiretroviral therapy (ART), elective cesarean section, and avoidance of breastfeeding have been implemented. This prospective cohort study aimed to assess the determinants of and the temporal trends in HIV-1 vertical transmission in the metropolitan area of Belo Horizonte, Brazil from January 1998 to December 2005. The rate of HIV-1 vertical transmission decreased from 20 percent in 1998 to 3 percent in 2005. This decline was associated with increased use of more complex ART regimens during pregnancy. Multivariate analysis restricted to clinical variables demonstrated that non ART, neonatal respiratory distress/sepsis and breastfeeding were independently associated with HIV-1 vertical transmission. When laboratory parameters were included in the model, high maternal viral load and non maternal ART were associated with HIV-1 vertical transmission. The results from this study confirm the impact of ART in the reduction of HIV-1 vertical transmission and indicate the need for improvement in the care and monitoring of mother and infant pairs affected by HIV-1.

Introdução: um crescente número de mulheres infectadas pelo HIV tem sido identificado, principalmente em idade reprodutiva. Apesar de receberem todas as medidas para a prevenção da transmissão vertical, observam-se dificuldades na assistência pós-natal, incluindo a ocorrência de gestações não-planejadas. Objetivo: investigar percepções sobre a contracepção por mulheres sabidamente infectadas pelo HIV com experiência prévia da maternidade e que engravidaram após o diagnóstico. Métodos: este foi um estudo clínico-qualitativo, desenvolvido no ambulatório Carlos Chagas do Hospital das Clínicas da Universidade Federal de Minas Gerais, de Janeiro de 2004 a Dezembro de 2005. Foram incluídas mulheres infectadas pelo HIV, com filhos vivos prévios e novas gestações após o diagnóstico. O número de entrevistadas foi definido pelo critério da saturação. Entrevistas semi-estruturadas foram gravadas e transcritas na íntegra. A análise foi realizada pelo processo de categorização. Resultados: vinte mulheres foram entrevistadas, com mediana de idade de 29 anos. A mediana de gestações foi de 3,5, mas após o diagnóstico 1,04. Dezenove gestações não foram planejadas. As mulheres tinham informações sobre métodos contraceptivos disponíveis, mas relatavam dificuldade do uso do preservativo pelo parceiro, dificuldades de uso de anticoncepcional oral e de acesso à salpingotripsia. Conclusão: a ocorrência de gestações não planejadas não dependeu do conhecimento prévio da infecção pelo HIV. As expectativas e o desejo podem ser modificados pelo estigma da doença, mas não foram determinantes na utilização do método contraceptivo eficaz. Profissionais de saúde devem atuar de forma integral, focando a utilização de contracepção eficaz e permitindo com que elas exerçam seus direitos reprodutivos.
